Student Application Procedure

The fillable PDF document will be emailed to you by the Registrar of Oklahoma Academy. Click here to request an application form. The application is designed to be completed on Adobe Reader and is accessible on ChromeOS, Android, Windows, iPadOS and MacOS. I. A completed and signed application form

The prospective student should fill in the information on pages 3 to 9 of the application.

The Parent/Guardian(s) should read and sign page 9 and fill in all of the requested information on pages 11 to 18 of the application.

All pages must be filled out before you return the application, or else there will a delay in processing.

III. An Official Transcript or recent Report Card

A. Home School students need to send us a complete transcript from the study program being used. If no transcript is available, please request a copy of our "Home School Credits Assessment Form". 

B. Home School students should also take the most current ITBS (Iowa Test of Basic Skills). THis can usually be taken at your local public school or ordered online.

V. Pay the Application Fee

The application fee for domestic applicants is $20.

The application fee for international applicants is $50.

This fee will be requested when the completed application is received.

The Consent to Treat form is another required from that must be turned in with the application. This form must be turned in by mail or on the day of registration. This is the only form that must not be emailed. Notice a notary must sign this document. This ca be done during the Registration Process.

Once the Completed Application is Emailed

  1. Evaluation of all forms will be conducted by the Registrar. Applicant will be informed of any incomplete information needed to have all forms necessary for the Acceptance Committee.

  2. An online video student interview will be conducted by the respective dean to the applicant.

  3. References will be contacted and these completed forms will be seen by the Acceptance Committe.

  4. Once a decision has been made, you will receive a letter with further instructions
