We are excited you have chosen to be part of Oklahoma Academy! We have a few steps to help make your enrollment as smooth as possible. If you are interested in our high school diploma please complete the following steps. Please refer to our application process to make sure you have everything you need. Application Form

1.  A completed and signed application form

The prospective student should fill in the information on pages 3 to 9 of the application form.

The Parent/Guardian(s) should read and sign page 9 and fill in all of the requested information on pages 11 to 18 of the application form ​

2.  Include a picture with your application.

​

3.  An Official Transcript of your most recent Grade Report Card

A.  Home school students need to send a complete transcript from the study program used. If no transcript is available, please request our Home School Credits Assessment Form.

B.  Take the most current ITBS Test (Iowa Test of Basic Skills). This can be ordered online.

4.  Send a copy of the prospective student's complete Immunization record.

For more information on what is required in regards to immunizations, please refer to pages 13 and 14 of the application.

5.  Pay your $50.00 application fee

 

The Consent to Treat Form is another required form that must be turned in with the application. This form is found on the website and must be turned in by email or on the day of registration. This is the only form that must not be emailed. Notice a notary must sign this document. This can be done during the Registration Process upon arriving at Oklahoma Academy.​

Application Process

1. Admission Committee will email the applicant and conduct an online video call interview

2. Admission Committee will call guardian with acceptance Notification

3. The student will be given an English & Math Placement Test.

Upon acceptance, order your uniforms.
